Care Management

While many people In New York State receive their health benefits through the Medicaid program, most people are generally healthy. For those friends, neighbors and loved ones who have chronic health problems, Jawonio Care Management is the life-changing and life-saving choice they need.

Qualifications

  • People 18 years old who are Medicaid Enrolled*
  • Have two chronic medical conditions
    – OR –
  • One single qualifying chronic condition:
    (Serious Mental Illness; HIV/AIDS; Sickle Cell Disease)

*Care Management may be available for members with a serious persistent mental illness that are not enrolled in Medicaid, please call for more information

About Care
Management

Jawonio Care Managers provide person-centered care coordination for individuals with serious mental health and/or chronic health challenges access the resources they need to successfully achieve their personal, educational, employment and wellness goals.

Jawonio Care Managers are dedicated to coordinating care among all providers
of medical and social services and help to:

  • Navigate the health care and social services system
  • Locate a doctor or provider for regular medical care
  • Provide referrals to community supports and services including temporary assistance, housing, SNAP (Supplemental Nutrition Assistance Program) and emergency food resources
  • Arrange transportation to medical appointments
  • Assist in maximizing health benefits (insurance, social security)
  • Apply for secure safe and affordable housing
  • Seek opportunities for employment
  • Facilitate case conferencing with doctors / providers and care team
  • Help to understand medications
  • Understand importance of appointments and reminders
