Summary:

In conjunction with the Division Director of CLS and Director of Day Habilitation, take a leadership role for all phases of admission to residential services including obtaining and review of referral documents, contact with referral sources including county and state representatives and family members.

Responsible for tracking and maintaining Social Security, Medicaid, Food Stamps and any other entitlement benefits for all IRA and Day Hab Participants. Complete re certification for all benefits and trouble shoot as necessary to resolve issues regarding individual’s benefits .

Assists the Division Director to ensuring that the needs of the families of individuals are addressed. Communicates or confers with individuals’ parents and/ or advocates addressing program, providing support, education and direction, responding to issues and questions, etc.

Monitor and ensure accuracy of CAS Assessments completed for individuals in CLS and Day Habilitation. Work with Care Management to make corrections and other changes as necessary.

Represents the Directors at county MR/MI work group, Collecting and sharing up to date information regarding the needs of individuals and of the county in providing those services.

Participates in a broad program of quality assurance regarding clinical indicators.

Liaison to community, county, state and other providers regarding MR/MI services to individuals to ensure open and clear communication, collaboration and documentation.

With division Director at a variety of county, state meetings and conferences to discuss agency activities and plans as a way of maintaining awareness of trends and activities throughout the rehabilitative field.

Education/ Experience

  • Master’s Degree in Social Work with LCSW-R designation or Master’s Degree in Counseling plus 10 years clinical/ counseling experience with disabled population. Current Knowledge of and experience in OPWDD, OMIG and other related federal and state funding regulations.
  • Knowledge of benefits and entitlements procedures and systems.
  • Strong clinical and organizational skills
  • Experience counseling and facilitating groups individuals with I/DD
  • Experience with facilitation family counseling and groups
  • Experience with person-centered planning approaches and positive behavioral supports preferred.
  • Experience with the process of residential admission to fill vacancies and for residential and day opportunities .
  • knowledge of applicable program standards and regulations.
  • Experience with developing and delivering training curricula.
  • Knowledge of policy development and implementation.
  • Proficiency in implementing and maintaining compliance standards
  • Knowledge of practices concerning investigations.
  • Knowledge of practices concerning investigations.

Additional Requirements:

  • Daily access to an automobile is required
  • a valid NY State Driver’s License and ability to drive and driving record acceptable for agency insurance coverage are required.
  • Must be cleared/ maintain acceptable record under NY state mandated criminal background check process
  • Must be cleared and maintain an acceptable record under the Medicaid Fraud & Abuse clearance process.
  • On- going training/ education as part of development related to position. programs are regulated by OMWDD, OMH, DDD, OCFS & DOH.
