It is the role of the Care Coordinator to ensure that an individual can go to a "one stop" facility and receive the services he requires or desires in a comfortable, secure and coordinated practice.
Experience shows, and research confirms, that individuals feel most comfortable receiving primary and other medical care and behavioral health services in a setting where everyone who works in that setting has an understanding of and commitment to comprehensive, inclusive and integrated health care.
A Care Coordinator is but one part of the larger Care Coordination team, which consists of the physicians, clinicians, nurses, dentists, outreach workers, care managers and MSCs, and the individual’s circle of support. It is the ultimate role of the Care Coordinator to pull all these together for the benefit of the individuals we are committeed to serve, in a respectful, person centered, mission driven organization.
Embedded in the Health Center at Jawonio is both grant funded, which serves all at-risk populations, and non-grant funded care coordination.
